Technical Assistance:
Discussion on appropriate documentation to verify proof of identity. A report card is not sufficient documentation.

Comments:
An unannounced Renewal inspection was conducted this date. 48 children on site with 3 staff. Observation of children transitioning from after school to the program location was made. Attendance was called and children were observed participating in both large and small groups. Emergency procedures, CPR/First Aid certification, daily health observation and emergency contacts were verified with staff. Activities consisted of legos, homework, stacking blocks and some electronic games. Snack was juice and crackers. Hand washing procedures and table sanitization was observed. All medication was reviewed. Note: Staff files are reviewed on an annual basis by representatives from the Commonwealth of Virginia, Department of Social Services, Division of Licensing, Fairfax Licensing Office and through the County of Loudoun, Department of Parks, Recreation, and Community Services? main office located in Ashburn, Virginia. Any observed standards found to be out of compliance would be cited in a violation notice issued at that time. This inspection took place between 2pm and 4:45pm. Thank you for your cooperation today. Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, 3 records were found incomplete. Evidence: Child #3 #4 and #5 did not have documented proof of identity that includes the reviewer.

Plan of Correction: We will ensure that staff verifies and documents proof of identity for each child enrolled.

Standard #: 22VAC40-185-260-A
Description: Based on observation and staff interviews, the center was unable to provide a copy of the annual fire inspection report. Evidence: The center is located in a public school and the inspections are maintained in the main office which staff was unable to locate the inspection.

Plan of Correction: We will provide a copy to the licensing office.

Standard #: 22VAC40-185-260-B
Description: Based on observation and staff interviews, an annual health inspection could not be located. Evidence: The center is located in a public school and the inspection is maintained in the main office and inaccessible to staff.

Plan of Correction: We will provide a copy of the inspection to licensing

Standard #: 22VAC40-185-510-D
Description: Based on review of all medications at the center, one medication authorization was not available. Evidence: Child #5 requires an EpiPen. This medication was not found on site.

Plan of Correction: Staff has requested this medication from the parents. Will follow up with parent when child is picked up today.

Standard #: 22VAC40-185-510-E
Description: Based on review of medications, one medication was not labeled as required. Evidence: Bendaryl for Child #6 was not labeled with the child's name.

Plan of Correction: We will label this medication today.
