A Subsidy Health and Safety inspection was conducted today to review supplemental health and safety requirements. A licensing Renewal inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice 48 children on site with 3 staff. Observation of children transitioning from after school to the program location was made. Attendance was called and children were observed participating in both large and small groups. Emergency procedures, CPR/First Aid certification, daily health observation and emergency contacts were verified with staff. Activities consisted of legos, homework, stacking blocks and some electronic games. Snack was juice and crackers. Hand washing procedures and table sanitization was observed. All medication was reviewed. Note: Staff files are reviewed on an annual basis by representatives from the Commonwealth of Virginia, Department of Social Services, Division of Licensing, Fairfax Licensing Office and through the County of Loudoun, Department of Parks, Recreation, and Community Services? main office located in Ashburn, Virginia. Any observed standards found to be out of compliance would be cited in a violation notice issued at that time. This inspection took place between 2pm and 4:45pm. Thank you for your cooperation today. Description: Based on observation and staff interviews, documentation of training was not on site. Evidence: Staff #1 did not have documentation of the pre service training.

Plan of Correction: This staff is a substitute and the certificate is in the main office. Subs will bring copies of the training at each site.

Standard #: VENDSUB-000-012-E-1
Description: Based on staff record review and staff interviews, CPR certification could not be verified. Evidence: Staff #2 did not have documentation of CPR certification on site.

Plan of Correction: We will maintain all certifications on site.
