Comments:
An unannounced monitoring inspection was completed on September 18, 2017. The purpose of this visit was to measure compliance with the licensing standards. There were 15 preschool aged children in care at the time of the inspection. There were 2 staff members working directly with the children in their classrooms. The children throughout the facility were engaged in activity. The children were participating in the morning work cycle at the time of the visit. Children were engaged in practical life work, puzzles, rectangle cards from the geometric shape cabinet and block work. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with Montessori manipulatives and supplies and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information. No medication was on site.

During the inspection, 5 children's records and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of a sample of staff records, the facility did not ensure that three staff persons obtained and submitted the results of a follow-up tuberculosis screening two years from the date of the most recent TB screening. EVIDENCE: The records of staff person #2, TB test date 3/26/15; staff person #3, TB test date 9/10/15; and staff person #4, TB test date 2/21/15, did not contain results of a tuberculosis screening that had been updated within 2 years of the date of the most recent screening.

Plan of Correction: We will ask the employees to have the test done within the next 48 hours so they will be on file by next Monday.
