Comments:
An unannounced monitoring inspection was conducted on September 19, 2017. Upon arrival to the facility, during the aftercare portion of the program, there were two staff present with 14 children. Children were doing various table top activities and having snack. Five children's records were reviewed on this date. Two medications were reviewed. Licensing Inspector reviewed supervision, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports.

Description: Based on review, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on September 19, 2017, the fire inspection report for the facility had expired and was dated 4/14/16.

Plan of Correction: Person in charge will check with the coordinator/supervisor to find an updated copy.
