Technical Assistance:
Reviewed all current and upcoming changes to the background check procedures. All communication regarding background check information will only be sent via email. Please move the portable teeter totter on the toddler playground. This piece of equipment needs to be placed completely on top of the resilient surfacing.

Comments:
An unannounced monitoring inspection was conducted today. There were 28 children in care, with a total of 7 staff members assigned to their direct supervision. Children's records, staff records, children's medications, the indoor and outdoor physical plant, emergency preparedness items, and administrative documents were all reviewed today. The children were observed playing with toys, engaging in creative play, eating lunch from home, and interacting with staff members. This inspection took place on the school picture day. The children were observed transitioning to and from their classrooms and the picture area. This inspection was completed between approximately 10:00 am and 1:00 pm. Violations:
Standard #: 22VAC40-185-80-A
Description: Based on a review of classroom attendance records, it was determined that one record was not completed accurately. Evidence: Upon entering the Infant classroom, the Inspector observed 6 children present and in care. However, the documented attendance indicated a total of only 5 children present and in care.

Plan of Correction: The attendance was immediately corrected during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observations, it was determined that there is not enough mulch in one fall zone on the playground. Evidence: There is not enough mulch in the fall zone surrounding the rock climbing wall. In this area, the mulch is at a level beneath 6 inches.

Plan of Correction: We will order and install additional mulch in that area.

Standard #: 22VAC40-185-370
Description: Based on staff interviews, it was determined that tummy time is not recorded as required. Evidence: The center does not maintain records of each infant's daily tummy time. The Inspector was unable to verify that each infant who requires tummy time is given at least 30 minutes each day.

Plan of Correction: Tummy time is already recorded on the daily sheets that are sent home with parents each day. I will create a tummy time log that is kept on site.

Standard #: 22VAC40-185-520-D
Description: Based on staff interviews, it was determined that staff do not record the application of insect repellent as required. Evidence: Staff applied insect repellent on four children during the summer months. It was reported to the Inspector that staff did not record the application of insect repellent.

Plan of Correction: Staff were immediately told to begin recording insect repellent when it is applied.

Standard #: 22VAC40-80-120-E-2
Description: Based on observations, it was determined that the findings from the most recent inspection were not clearly posted. Evidence: The violation notice from the most recent inspection was hanging beneath other paperwork on the parent board.

Plan of Correction: Today's violation notice will be clearly posted.
