Comments:
Today an investigation into a self-reported incident was conducted in the Preschool program as a result of a child found in classroom unsupervised. After interviewing staff, reviewing the incident report and face to name procedures, it was determined that the child was unaccounted for and not within sight and sound supervision for approximately 10 minutes. Standard #: 22VAC40-185-340-A
Description: Based on interviews with Staff, it was determined staff failed to ensure care, protection and guidance while supervising children. Evidence: On Tuesday September 9th, 2017, while in transition for outside playChild #1 was able to enter in the bathroom without staff being aware. At approximately 4:15pm Child #1 was observed standing in the middle of the the classroom. Staff #2, who was passing by the classroom en route to the playground. It was at this time that Staff #2 discovered that the child had been left in the classroom alone. Child #1 was left alone and unsupervised outside for at least 10 minutes without staff aware that the child was missing.

Plan of Correction: The day that the licensing inspector arrived we held an all-staff meeting to discuss the violation. We have added a transition column to the ratio sheet that the teachers use on a daily basis. We have also identified new locations during transitions to re-count students. Finally, our October staff meeting is dedicated to reviewing our Operations Manual and all of our daily procedures.

Description: Based on interviews with Administrators and Staff, it was determined staff failed to provide sight and sound supervision for a child under 10 years of age. Evidence: On Tuesday September 9th, 2017 at approximately 4:05-4:!5pm Child #1 was found in a classroom alone without a staff member. The staff did not realize Child #1 (3 years old) had been left in the classroom while the classroom went out to the playground. Child #1 was out of sight and sound supervision for at least 10 minutes.
