An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 1:40 PM and departed at 3:30 PM. Sixty-four children were in care with nine staff. Nap time, snack time and afternoon activities were observed. A sample size of six children's records were reviewed. Staff records were not reviewed as they are located at the main office. Medication administration was reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.
Standard #: 22VAC40-185-130-A Description: Based upon review of six children's records and staff interview, the facility has not ensured that they obtain documentation of required immunizations prior to the child attending. Evidence: 1. The record for child #3, in care during today's inspection, does not include documentation of immunizations. 2. Staff were unable to produce documentation of child #3's immunizations. Plan of Correction: The facility responded with the following: The immunization record will be added to the child's file. All records will be checked to ensure that required documentation is in each child's file.
Standard #: 22VAC40-185-60-A Description: Based upon review of six children's records, the facility has not ensured that each child's record includes documentation that required information has been reviewed and updated with parents at least annually. Evidence: 1. The most recent update documentation in the record for child #1 is dated 8/24/2016. 2. The most recent update documentation in the record for child #2 is dated 4/18/2016. Plan of Correction: The facility responded with the following: Update packages have been sent home to the parents of returning children. Those parents who have not returned the documents will be contacted and required to update the forms.
