Technical Assistance:
*Staff under the age of 18 must be under the sight and sound supervision of an adult staff member. **VA Pre-service training can be counted as annual training one time. Please notify your inspector of suspension or termination of services for more than 24 hours as a result of an emergency. Please notify your inspector ?preferably in writing ? of any serious injuries sustained at the facility. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment.

Comments:
An unannounced subsidy health and safety inspection (SHSI) was conducted with the center director and center staff. A licensing inspection was also conducted on this date (see that report for details). There are currently 13 staff employed by the facility. All staff have documentation of the mandatory VA Pre-service Training and most staff are currently certified in first aid and CPR. There are four children with diagnosed food allergies currently enrolled. The emergency preparedness plan was reviewed for reunification plans and lockdown procedures. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. Standard #: VENDSUB-000-012-E-1
Description: Based on review of thirteen staff files, the facility failed to have current CPR certification for three staff. Evidence: Staff A's CPR expired 8/26/2017, Staff C's CPR expired 8/28/2017, and Staff D did not have documentation of CPR certification.

Plan of Correction: Center director will schedule a CPR certification class for staff who need it.

Standard #: VENDSUB-000-012-E-2
Description: Based on review of thirteen staff files, the facility failed to have current first aid certification for three staff. Evidence: Staff A's first aid certification expired 8/26/2017, Staff C's first aid certification expired 8/28/2017, and Staff D did not have documentation of first aid certification.

Plan of Correction: Center director will schedule a first aid certification class for staff this coming month.
