The licensing inspector conducted an unannounced monitoring inspection, at which play groups, outdoor time, story time, activities, group time, pre-lunch hygiene time, lunch time, post lunch hygiene time, and rest time were observed. Children were observed; and staff and children files and other documentation were reviewed. Fifty-eight (58) children, aged 1 month-4 years, were found to be in care with 10 staff, kitchen staff, and administrative staff. Previous violations were reviewed, and one (1) of the violations from the previous inspection had not been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 9:30am and concluded at 1:00pm.
Standard #: 22VAC40-185-60-A Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to completion of enrollment documents. Non-compliance was previously cited on 9/29/16, 4/26/17, and again today 9/29/17. Therefore this violation is marked as repetitive and as a system deficiency. Evidence: Previously cited child record #1 still did not have the addresses for the listed emergency contacts as required. Child record #2 did not address any allergies or chronic physical problems as required. Plan of Correction: Administrative staff will do a full record review to ensure all are in compliance with standards. Upon enrollment, records will be verified by administrative staff for completion as required.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records the facility failed to maintain compliance with this standard as required relating to required staff documents. Evidence: Staff record #1, hire date 9/5/17, did not contain an emergency contact, and did not list any physical/medical issues as required. Staff record #2, hire date 9/5/17, did not contain proof of education/qualifications for the position held as required. Plan of Correction: Staff files will be reviewed by administrative staff for compliance. Missing items will immediately be placed within the files for compliance. Upon hiring ,staff files will be reviewed for compliance.
