Comments:
An unannounced monitoring inspection was conducted on September 29, 2017. There were five children present with two staff members. Additional staff arrived during the inspection for administrative and support purposes. The children were observed during outdoor play, having lunch and napping. The following items were reviewed as a part of the inspection: nutrition, health and safety, indoor areas, required documentation, and emergency supplies, procedures and required postings. Five children's records and nine staff records were reviewed. Two medications were reviewed. The center is administering medication and offering transportation. The exit interview was conducted with the Owner/Program Director.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, in one of nine staff records reviewed, the licensee did not ensure that staff resubmit TB test results every two years. Evidence: During the inspection conducted on September 29, 2017, the TB screening for Staff #3 was more than two years old and was dated 7/12/15.

Plan of Correction: Owner contacted the staff member and will have the staff to update the TB screening.

Standard #: 22VAC40-185-70-A
Description: Based on record review, in one of nine staff records reviewed, the licensee did not ensure that each staff record was documented with all required information. Evidence: During the inspection conducted on September 29, 2017, there was not a name, address and phone number of a person to contact in the event of an emergency in the record for Staff #7.

Plan of Correction: Owner will contact the staff member for the emergency contact information.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on September 29, 2017, the licensing inspector observed a can of air freshener in the bathroom that was labeled caution and keep out of reach of children.

Plan of Correction: The air freshener was locked during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. Evidence: During the inspection conducted on September 29, 2017, the licensing inspector observed a plastic blue/green play composite with a slide attached that was positioned on Astroturf.

Plan of Correction: We will close that piece of equipment until we decide where it will be moved.
