Comments:
The licensing inspectors conducted an unannounced renewal inspection from 9:40am - 12:45pm. There were 90 children in care with 19 staff. The children were observed playing in the centers, at story time, playing on the playground and during lunch. Nine staff's records and nine children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observations, the center failed to have all hazardous substances stored in a locked place. Evidence: 1. The inspector observed the school age building. There was a container of Pine Sol in an unlocked cabinet under the sink.

Plan of Correction: The container was moved to a locked place during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center failed to have 6 inches of resilient surfacing under and around equipment on the school age playground. Evidence: 1. The inspector observed the playground and measured the resilient surfacing. There was 3 to 4 inches of mulch at the end of the slides and surrounding the fireman's pole. The fall height of the equipment is 48 inches.

Plan of Correction: The director will order additional mulch for the playground. The children will not use this equipment until additional mulch is added to these areas.
