A physician plan of action for allergies needs to include a plan for suspected reaction and/or confirmed reaction for each food item.
A Subsidy Health and Safety inspection was conducted on October 2, 2017 to review supplemental health and safety requirements. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Standard #: VENDSUB-000-006-B-11 Description: Based on record review of 5 children's files that have listed allergies, the center has not obtained written instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected and/or confirmed allergic reaction for all of them. Evidence: 1. Child 1, 2, 3, and 4 have listed food allergies but there are no written plan of actions from physicians. 2. Child #1 has a long list of foods but these foods are not on the physical report. The report has nuts but not the different shellfish listed. Plan of Correction: Not available online. Contact Inspector for more information.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
