Today an unannounced monitoring inspection was conducted between the hours of 2:00pm and 3:45pm. I observed 29 children with 3 direct-care staff. Ratios and supervision were in compliance. Children were observed arriving to the program, washing hands, eating snack, and playing in the playground. Staff were actively engaged with the children and assisting them with tasks as needed. Snack served: Capri Sun and chocolate chip crisp. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Medication management was in compliance. Required staff training documents were on-site. Emergency preparedness supplies in order. Required documents posted. Record keeping was in compliance. Standard #: 22VAC40-185-550-L Description: Based on record review, documentation of a child's injury report was found to be incomplete. Evidence: An injury report for child#1 was not filled out properly, the date or time of parent's notification were not documented. Plan of Correction: I will rewrite the form and have the parent sign it.
