Comments:
An unannounced subsidy Health and Safety Monitoring inspection was conducted from 1:15pm until 4:30pm. There were 28 children, ages 2 to 4years old in care at the time of arrival with 4 staff. The records sample size consisted of 5 children?s records and 6 staff records. Children were observed in teacher and self-directed activities. There are no children currently enrolled who have diagnosed food allergies. Areas of non-compliance are identified on the violation notice. Thank you for your time during today?s inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-012-E
Description: Based on record review and interview the vendor failed to ensure that all staff providing care to children have within 30 days of the date of employment CPR training. Evidence: 1 - The record for staff 2 does not contain documentation of CPR or first aid training. a. The documented date of hire for staff 2 is 8/28/17. 2 - Staff 1 confirmed that there is not any documentation of CPR or first aid training in the record of staff 2.

Plan of Correction: Going forward center management will ensure that all staff obtain the required CPR and first aid training within 30 days of employment.

Standard #: VENDSUB-000-012-E-1
Description: Based on record review and interview the vendor failed to ensure that current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. The training shall include an in-person competency demonstration Evidence: 1 - The documentation provided regarding CPR training for staff 3 did not include an in-person competency demonstration. 2 - Staff 1 confirmed that the documentation of CPR training for staff 3 did not include an in-person competency demonstration.

Plan of Correction: Going forward center management will ensure that all CPR training includes the required in-person competency demonstration.

Standard #: VENDSUB-000-031-A
Description: Based on review of emergency procedures and interview the center failed to ensure that there are written procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child. Evidence: 1 - There are not any written procedures available for review that address reunification after an emergency evacuation. 2 - There are not any written procedures available for review that address reunification after a shelter-in-place emergency. 3 - There are not any written procedures available for review that address reunification after a lockdown emergency.

Plan of Correction: Center management will create, write, implement and train staff in all reunification procedures.
