- Technical Assistance:
During the inspection, licensing inspector discussed with center supervisor upcoming trainings set to expire at the end of the month and children's emergency medications.
An unannounced monitoring inspection was conducted from 7:45am-9:20am. At the time of entrance, 26 children were in care with 2 staff members present. Children were observed during engaged in group activities such as puzzles, games, crafts, and reading. Staff were positively interacting with the children who were in return, very responsive and engaged. A selection of 5 children records, medications and emergency plans were reviewed. Standard #: 22VAC40-185-140-A Description: Based on a sample review of children's records, the center did not ensure that children's files contained a physical within 30 days of enrollment. Evidence: The record for Child #1 [enrolled 8/30/2017] was missing a physical examination. Plan of Correction: I'll contact the main office and speak with the mother.
Standard #: 22VAC40-185-40-D Description: Based on observation, the center did not ensure that the current license was posted in a conspicuous place. Evidence: Though a current license has been issued, the license posted during the inspection expired May 2017. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method. Evidence: 1. Several cans of spray paint and corking glue were observed in an unlocked plastic bin. 2. Several cans of spray paint were observed in an upper unlocked cabinet. Plan of Correction: A work order has been placed for the disabled locked on the upper cabinet. The spray paint in the plastic bin will be locked in a cabinet.
Standard #: 22VAC40-185-420-E-3 Description: Based on a sample review of records, the center did not ensure to obtain parent confirmation that the required information in the child's record is up to date annually. Evidence: 1 of 5 children's records reviewed were missing an annual update of information contained in the record. (Child #3; enrolled 10/2/2016). Plan of Correction: I'll have the parent update the information.
