An unannounced Subsidy Health and Safety Inspection was conducted on October 13, 2017 from approximately 12:25 p.m. until 2:30 p.m. There 46 children in care and 4 staff present with children. The record review sample consisted of 5 children's records and the on site required documentation for 5 staff. Children were observed preparing for and eating lunch. Children were also observed playing large group games and in centers to include legos, board games and dolls. The results of the inspection were reviewed with the center director. Standard #: VENDSUB-000-006-B-11
Description: Based on documentation review and staff interviews, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on October 13, 2017, the record for child #1 has documentation of a diagnosed food allergy (soy, milk, wheat, eggs, corn and sesame seeds) and there is not a written allergy care plan.

Plan of Correction: The center director has given mom the information and is waiting to get the form back.

Description: Based on staff interviews, the vendor did not ensure staff had obtained orientation training on facility specific topics prior to the staff member working alone with children and within seven days of the date of employment or the date of subsidy vendor approval. Evidence: During the inspection on October 13, 2017, staff # 3 and staff #5 confirmed they have not had training related to food allergies and emergency preparedness specific to the center. The center staff and center director verified this information.

Plan of Correction: The center director will set a meeting with part time staff and make sure they know where everything is and what to do in an emergency.

Standard #: VENDSUB-000-012-E
Description: Based on observation and interviews, the vendor did not ensure all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval CPR and first aid training. Evidence: During the inspection on October 13,2017, staff #3 and staff #5 did not have documentation of obtaining CPR and first aid training. Staff #3 verified they began working in 2016 and staff #5 April 2017 verified they began working . The center director verified the information.

Plan of Correction: The center director said all part time staff will have the training by the end of the year.
