Comments:
An unannounced monitoring inspection was conducted in the Center on October 2, 2017. The Licensing Inspector observed 20 children and 4 staff present in the Center. The children were 3 and 4 years old. Reportedly, there are 29 children currently enrolled. The children were observed in two classrooms, 2 staff members were supervising each group. The LI observed the children during self-initiated centers play, free play outdoors on the play ground, nap/rest time, afternoon snack and dismissal procedures at the end of the program day. The physical plant was maintained clean, orderly and supplied with age appropriate materials. A sample of 5 children's records was reviewed. Medications were observed for one child and one staff. Three staff present at the time of the inspection have completed medication administration training (MAT). The center's first aid kit was complete with the required items. One partial staff record was reviewed on site. Five complete staff records were reviewed at the central office on September 29, 2017 prior to the monitoring inspection. Findings were reviewed at exit with the Supervisor of Monitoring and Compliance. The on-site inspection was conducted from approximately 11:00AM to 1:50PM. Description: Based on reviewed documentation, the Center failed to ensure the Search of the Central Registry for staff was verified before 30 days of employment at the facility ended. Evidence: The LI reviewed five staff records, one of five (staff 2) did not contain a verified Search of the Center Registry before 30 days of employment at the facility ended. Date of employment for staff 2 was noted as 08/21/17; records were reviewed on 09/29/17. Written notation on the form indicated the form was mailed on 08/29/17; a follow-up made on 09/25/17.

Plan of Correction: The Center will submit all background requests within the first 7 days of employment. Follow-up will be made by the 30th day of employment. The verified background check will be submitted to the Licensing office for review.
