Technical Assistance:
*Staff under the age of 18 must be under the sight and sound supervision of an adult staff member. **VA Preservice training can be counted as annual training one time. Please notify your inspector of suspension or termination of services for more than 24 hours as a result of an emergency. Please notify your inspector ?preferably in writing ? of any serious injuries sustained at the facility. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment.

Comments:
An unannounced subsidy health and safety inspection (SHSI) was conducted with the center director and center staff. A licensing inspection was also conducted on this date (see that report for details). There are currently 3 staff employed by the facility. All staff have documentation of the mandatory VA Preservice Training and are currently certified in first aid and CPR. There are 4 children with diagnosed food allergies currently enrolled. The emergency preparedness plan was reviewed for reunification plans and lockdown procedures. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on review of five children's records, the facility failed to have a written allergy action care plan for each child diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction. Evidence: Child 1 and Child 3 did not have documentation of a written allergy action plan for diagnosed food allergies.

Plan of Correction: Center director will ask the parent to provider a written allergy action plan completed by a physician.

Standard #: VENDSUB-000-031-A
Description: Based on review of the emergency preparedness plan, the facility failed to have all components required for the emergency preparedness plan. Evidence: there were no written procedures on the reunification of children with parents after an evacuation or after a shelter in place drill. There were no written procedures to accommodate children with special needs to ensure their safety during evacuation or relocation.

Plan of Correction: Center director will check with corporate to see if there are updates to the emergency preparedness plan.

Standard #: VENDSUB-000-031-B-3
Description: Based on review of the emergency preparedness plan, the facility failed to have written lockdown procedures to include facility containment. Evidence: there were no written procedures for methods to alert staff and children of lockdown, methods to secure the facility, methods to account for all children, methods of communication, accommodating children with special needs, or reuniting the children with parents.

Plan of Correction: Center director will check with corporate office to see if there are updates to the emergency preparedness plan.
