An unannounced monitoring inspection was conducted from 11:05 a.m. - 2:55 p.m. There were thirty-eight children in care with seven teachers working directly with them. There was another staff person available to assist as needed. Six children's records were reviewed. The record for five staff were reviewed. The children were observed during hand washing, lunch, singing, reciting verse, saying grace, talking with teachers and one another, playing with toys and engaged in games. The younger children were observed during lunch, being freshened-up, being rocked and comforted by staff, playing, babbling, talking and during nap. Standard #: 22VAC40-185-160-A Description: Based on a review of five staff records the facility failed to have documentation of a negative tuberculosis test/screening for one staff member within twenty-one days of employment. Evidence: 1. On 10/18/2017 the licensing inspector review the record for staff #2. There was no documentation of a negative tuberculosis test/screening within twenty-one days of hire. The hire date for staff #2 was August 21, 2017. 2. On 10/18/2017 staff #1 confirmed there was no documentation of a negative tuberculosis test/screening in the record for staff #2. Plan of Correction: Staff #2 did provide from the health department a negative tuberculosis screening for the record. The document apparently has been misfiled. We have already notified the local health department that this document is needed for our records. We should have this by Monday.
