Comments:
An unannounced monitoring inspection was initiated today at 11:50am. The physical plant, 5 staff files, 5 children's files, annual fire inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 40 children under the supervision of 5 staff. During the inspection, children were observed preparing for and eating lunch, enjoying free play and listening to a teacher read a book. For lunch, the children were served breaded chicken nuggets, mixed vegetables, cantaloupe and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 2 staff certified in MAT (medication administration training). Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observations, there were areas of the center which were not maintained in a safe condition. Evidence 1. There were several wooden boards that had become detached from the playground fence, which had protruding nails.

Plan of Correction: The director will have someone come in to fox this.

Standard #: 22VAC40-185-510-C
Description: Based on observations, there were medications at the center without all of the required documentation. Evidence 1. Child #1 has Benedryl on site in case of an emergency. The authorization form (completed in April) does not include the duration that the medication is to be authorized or a physician's signatures. 2. Child #2 has Benedryl on site in case of an emergency. The authorization form does not include a physician's signatures.

Plan of Correction: The Director will ask the parents to complete new forms.
