Comments:
The licensing inspector for Bears and Blankets Academy for Early Learning conducted an unannounced complaint inspection. Observations of the facility and programming and interviews with staff were conducted relating to the allegations regarding physical plant, ratios and staff qualifications. The information gathered during the investigation does support the allegation, so the complaint is determined to be "valid." Standard #: 22VAC40-185-270-A
Complaint related: Yes
Description: Based on observation, the administration failed to ensure all areas and equipment of the center are maintained in a clean, safe and operable condition.

Evidence:

1. Rest mats were observed torn on the edges exposing the absorbent sponge underneath.
2. On the back wall in the basement and in the infant room paint and drywall were peeling from walls.

Plan of Correction: All mats will be looked at immediately and the ones that are torn will be replaced. The walls will be repaired.

Complaint related: Yes
Description: Based on observation, the administrator failed to ensure two crib mattresses had no more than one inch between the mattress and the crib. Evidence: Two of the eight crib mattresses had at least 1 1/2 inches between the mattress and the crib.

Plan of Correction: The mattresses will be replaced.
