An unannounced monitoring inspection was conducted on October 12, 2017. There were 73 children and 13 staff members present with children. Additional staff were on site for administrative and program support. The sample size consisted of 8 staff and 7 children's records. Three medications were observed and documents inspected. Children were observed during reading time and preparing for and eating lunch. The infants were observed freely playing and eating. The playground was not inspected due to inclement weather. Areas of non-compliance are identified on the violation notice. The inspection began at approximately 7:55 a.m. and ended at approximately 12:45 p.m. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a copy to the licensing office within 5 days of receipt. Standard #: 22VAC40-185-60-A Description: Based on record review and staff interviews, the licensee did not ensure there was at the center a separate record for each child enrolled which shall contained all the required information. Evidence: During the inspection on October 12, 2017 seven children records were reviewed and two (child #6 and child #7) 7did not include the employment phone numbers for one of the parents listed. Plan of Correction: This information was requested from the parent the day of the visit. All new enrollees will provide this information in full prior to starting enrollment, with center management at a paperwork appointment before the child's start date.
Standard #: 22VAC40-185-560-F Description: Based on measurements and staff interviews, the licensee did not ensure that when the center choose to provide meals or snacks the centers shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: During the inspection on October 12, 2017, the USDA guidelines were not followed while serving lunch to the 16 children in the 3 to 4 year old room. Fourteen children were served 1/2 cup of milk and 3/4 cup of milk is required by the USDA. Two of the sixteen children were not served milk at all with their lunch because the center staff said they do not drink milk. Another alternative was not offered to the children. The two children were not served all of the required four food group during lunch as the fourth food group was milk. Lunch consisted of pears, peas and carrots, cheese quesadillas and milk. Plan of Correction: Staff (including school food specialist) will all be retrained on USDA standards on or before 10/27/17. Management will make it a point to observe meal times daily to ensure compliance. DM will also observe meal service at next school visit.
Standard #: 22VAC40-185-560-K Description: Based on observation and staff interviews, the licnesee did not ensure that tables were sanitized before and after each use for feeding. Evidence: During the inspection on October 12, 2017, the table in the toddler 1 room was observed being sprayed only with a soap and water solution after feeding and it was not sanitized. This was confirmed by the center staff. Sanitized is defined as treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution. Plan of Correction: Staff will all be retrained on table sanitation prior to meals, on or before 10/19/17. Management and Safety Captain will both observe daily for compliance.
