Technical Assistance:
An exit meeting was completed with the licensing inspector and the center's assistant director to review the inspection outcome.

The licensing inspector conducted an unannounced monitoring inspection on Monday, October 16, 2017 from 10:55 am to 1:03 pm to determine the center's compliance with licensing standards. During this inspection, the assistant director was present and assisted the licensing inspector throughout this inspection. The census for today's inspection consisted of 46 children in care. The licensing inspector completed observations in the various classroom settings. The infants were observed by the licensing inspector completing a variety of activities to include sleeping in designated cribs, playing with age appropriate toys on the floor, interacting with their peers and staff. The staff were observed by the inspector completing feedings and diapering of the infants. The diapering station was deemed appropriate and in compliance with licensing standards. The Toddlers were observed by the inspector in the classroom engaged in free play time with a variety of age appropriate toys, interacting with peers, and the staff read a "Pajama time" story on the carpet area to the children. The licensing inspector observed the children in the Twos classroom sitting at the table eating their lunch with their peers and staff. The center provides both am and pm snacks only and the parents provide their children's lunch. The children's snack for today consisted of cheez-its and bananas. A formal observation was completed by the inspector of the children in the Threes classroom in which they were participating in circle time and the staff engaged in an interactive activity identifying various colors and shapes. The inspector also completed observation of the staff preparing for lunch time in the classroom. The JK children were observed by the licensing inspector participating in centers and completing an arts and craft activity for Halloween. The inspector observed a variety of activities completed by the children in the classroom to display for the center's upcoming Fall festival. The inspector interacted with the staff and children throughout the inspection when appropriate. The classrooms were equipped with age appropriate toys, and supplies. The staff were observed providing adequate supervision and addressing the children's overall needs. During this inspection, the required postings were reviewed and determined to be in compliance. The following information was reviewed: daily attendance, snack menu, license, recent inspection summary, fire and shelter-in-place drills, and liability insurance policy. There were five children?s records and six staff's records reviewed during this inspection. There are multiple staff with current MAT, CPR, First Aid and Daily Health Observation training on site. Medications are currently being administered at the center. The center provides transportation and the three buses/van were inspected as well. The center's first aid kit and emergency supplies were inspected.

Standard #: 22VAC40-185-160-A
Description: Based on review of staff record, the center did not ensure documentation of tuberculosis screening was submitted no later than 21 days after employment. Evidence: In staff#2's (date of hire: 09/05/17) record, the documentation of a tuberculosis screening was not found. Documentation of the screening shall be submitted no later than 21 days after employment.

Plan of Correction: Staff will ensure that staff's documentation of tuberculosis screening is obtained as soon as possible and a copy is included in the staff record.

Standard #: 22VAC40-185-270-A
Description: Based on observation of areas of the center, the center did not ensure all inside areas were maintained in a safe condition. Evidence: During this inspection, the licensing inspector observed peeling paint on the lower wall area in the Toddler classroom nearby the diapering station and this area was accessible to the children in care.

Plan of Correction: Staff will contact maintenance to address peeling paint areas of concerns.
