Comments:
Today an unannounced Monitoring Inspection took place between the hours of 3:45pm and 5:00pm. At the start of the inspection there were a total of 26 children with 3 staff; all ratios were in compliance. A total of 5 children?s files were reviewed. The children were observed working on homework and other organized activities. Violations were found during today's inspection. The violation notice will be printed and posted as soon as possible. Violations:
Standard #: 22VAC40-185-510-A
Description: Based on observation and interview, the center failed to obtain a written authorization from a child's parent to administer medicine. Evidence: Child #2 had medication on site for a food allergy. There was no written authorization to administer the medication from a parent.

Plan of Correction: We have obtained this form from the parent.

Standard #: 22VAC40-185-510-C
Description: Based on observation and interview, the center failed to obtain a long-term medication authorization from a child's physician. Evidence: Child #1 has a long-term medication authorization to administer a medication that was signed by the parent, not the child's physician, as required.

Plan of Correction: We will send this medication home to the parents.

Standard #: 22VAC40-185-550-L
Description: Based on record review, minor injury reports did not contain all of the required information. Evidence: Two out of four injury reports were missing the time that the parents was notified of the child's injury.

Plan of Correction: I'll check behind my leader.
