Technical Assistance:
The fenced toddler/two's playground will be measured to include added play area. Please submit drawing/measurements to the LI.

Comments:
An unannounced Renewal inspection was conducted in the Center on October 18, 2017. Upon arrival at approximately 10:30am, the LI verified 51 children in attendance. The children were in 4 separate groups. Each classroom met staff to child ratio. The children were observed as they participated in varied activities during the program day. The activities included free play, teacher-directed, outdoor play, lunch, nap/rest time, afternoon snack, and dismissal procedures at the end of the day. Staff were observed interacting with the children. The LI observed the Teacher in the two-year old classroom reading to the children. The physical plant was clean and orderly. Each classroom was supplied with age appropriate materials. A sample of 8 children's records was reviewed. Medications for 3 children were observed. Twelve staff records were reviewed which included all staff employed after the inspection date of June, 2017. The Center's emergency policies and documentation of the practiced emergency drills were reviewed. Documentation of the Center's annual fire and health inspections was current. Violations:
Standard #: 22VAC40-185-290-3
Description: Based on direct observations, the Center failed to ensure protective covers were on each electrical outlet. Evidence: The LI observed 1 electrical wall outlet uncovered in the two-year classroom. The outlet was not within reach of the children standing on floor level.

Plan of Correction: Classroom staff covered the outlet upon notice. Staff will monitor on-going to ensure outlets are covered when not in use.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on staff records reviewed, the Center failed to ensure a background check was obtained/verified before 30 days of employment at the facility ended. Evidence: One of 12 reviewed records (staff #8) did not contain a verified Search of the Central Registry, staff's date of hire was recorded as 08/14/17.

Plan of Correction: The Program Director will submit the completed form to the appropriate agency immediately. The PD will inform the licensing office once the background check is received back by the center.
