Comments:
An unannounced monitoring inspection was conducted today. There were 47 children in care, with a total of 5 staff members assigned to their direct supervision. This inspection took place during the before school program. The children were observed decorating and sewing pillows, making pot holders, reading books, engaging in small group classroom activities, and playing in the gym. Dismissal at the start of school hours was also observed. Children's records, required on site staff information, children's medications, emergency preparedness items, and the physical plant were inspected today. This inspection was completed between approximately 8:00 am and 9:10 am. Thank you for meeting with me today. Description: Based on a review of required on site staff information, it was determined that three documents were not available. Evidence: The emergency contact information for three staff members was not available on site.

Plan of Correction: All three staff members will fill out a new emergency contact form today.
