Comments:
An unannounced monitoring inspection was completed on 10/23/2017 from 9:45am and ended at 12:00pm. At the end of the inspection there were 13 children in care with 8 staff. Children were observed eating lunch, participating in age appropriate activities and playing outside. The sample size was 5 children's records and 5 staff records where reviewed. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed at the exit interview.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of documentation and interviews, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each staff that contains all required information. Evidence 1. The record for staff #4 did not include the written information to demonstrate that the individual possesses the education required by the job position. 2. Staff #1 (Program Director) confirmed the records for staff #4 was not complete.

Plan of Correction: The facility responded: Staff #4 has completed high school and will bring in a copy of her high school diploma.
