Technical Assistance:
Consultation was given on background requirements that went into effect as of July 1, 2017.

Comments:
An unannounced monitoring visit was conducted today from 11:25-12:20. Fourteen children were present with two direct care staff which is within ratios for supervision. Children were observed playing on the community center playground (wearing yellow shirts to differentiate from neighborhood children), transition into the center, hand washing and reading time. The letter of the week is "E" and activities focused on E and seasonal art projects. Two medications were on-site with one staff having MAT certification. Staff records are maintained off site at a central location and were reviewed September 2017.. Areas of non-compliance are identified in the violation notice. Description: Based on record review, 1 of 2 children's records was incomplete. Evidence: Child #1 was missing two emergency contact addresses.

Plan of Correction: Center Director has requested information from parents and will continue to ask for information.

Standard #: 22VAC40-185-290-3
Description: Based on observation, an unused outlet was missing a protective cover. Evidence: The outlet by the CD player was missing a protective cover.

Plan of Correction: Outlet cover will be replaced immediately.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the most recent inspection notes were not posted. Evidence: The posted inspection notes were dated 8/8/2016. The most recent inspection was 1/24/2017,

Plan of Correction: Today's inspection report will be posted immediately.
