Comments:
Today's visit was a routine monitoring inspection. The inspection began at 9:45am and concluded at 4:00pm. There were 117 children present between the ages of two and five years with eighteen staff members and an additional twenty-four children arrived after school with three staff members. During the inspection all nine classrooms in operation during the inspection were viewed, snack time was observed, nap time was observed, both outside play areas were viewed, a bus was viewed, two medications were viewed, emergency supplies were viewed, arrival of school age children was viewed, eleven out of two hundred children's records were viewed, four out of thirty-eight staff records were reviewed and some policies and procedures were reviewed. Please provide evidence regarding background check requests within ten days of today's inspection and documentation of the date(s) of the background checks completion for missing background checks cited during today's inspection. Discussion took place regarding new background check requirements. The PD stated the center is no longer a vendor for subsidy. Violations:
Standard #: 22VAC40-185-130-C
Description: Based on the inspector's review of eleven out of two hundred children's records, a record did not have an updated immunization record between the child's fourth and sixth birthday. Evidence: The record for child #9 has an immunization record date 8/20/12. Based on the child's date of birth they are currently seven years of age and the immunization record is dated when the child was two years of age. Immunizations are required to be updated between the chid's fourth and sixth birthday.

Plan of Correction: An updated immunization record will be requested from the parent. The school age records will be audited.

Standard #: 22VAC40-185-510-G
Description: Based on the inspector's review of two prescription medications, the center failed to ensure one of the medications was maintained in its original labeled container. Evidence: The inspector viewed a prescription emergency inhaler, in a ziplocc bag, with a prescription label on the bag. The medication was no longer in the original box as dispensed from the pharmacy.

Plan of Correction: The PD will ensure all future medication are in their original labeled container in order to be accepted by the center.

Standard #: 22VAC40-191-60-B
Description: Based on the inspector's review of four out of thirty-eight staff records, the center failed to ensure signed sworn statements were completed before employment for a portion of the sample. Evidence #1: The record for staff member #1 has a documented date of employment as 9/1/17 and the signed sworn statement was dated 9/5/17. Evidence #2: The record for staff member #3 has a documented date of employment as 9/1/17 and the signed sworn statement was dated 9/5/17.

Plan of Correction: Future staff will complete sworn statements before employment is offered to the potential staff member.

Standard #: 22VAC40-191-60-C-1
Description: Based on the inspector's review of four out of thirty-eight staff records, the center failed to ensure the criminal record check was received within thirty days for a portion of the sample. Staff member #4 has a documented date of employment dated 1/4/17 and there is and there is no documentation of a completed CRC background check or evidence of follow-up with the agency responsible for conducting the background check to date. The staff member has been employed for more than thirty days.

Plan of Correction: The PD will conduct follow-up and document the results of the follow-up by the next program day. New CRC background check requests will be made if needed. The PD will provide evidence and results of the follow-up to the licensing inspector within ten days of today's inspection.

Standard #: 22VAC40-191-60-C-2
Description: Based on the inspector's review of four out of thirty-eight staff records, the center failed to ensure the search of the central registry was received within thirty days for a portion of the sample. Evidence #1: Staff member #1 has a date of employment indicated as 9/1/17 and there is no documentation of a completed CPS background check or evidence of follow-up with the agency responsible for conducting the background check to date. The staff member has been employed for more than thirty days. Evidence #2: Staff member #4 has a documented date of employment dated 1/4/17 and there is and there is no documentation of a completed CPS background check or evidence of follow-up with the agency responsible for conducting the background check to date. The staff member has been employed for more than thirty days.

Plan of Correction: The PD will conduct follow-up and document the results of the follow-up by the next program day. New CPS background check requests will be made if needed. The PD will provide evidence and results of the follow-up to the licensing inspector within ten days of today's inspection.

Standard #: 63.2-1720.1-B-3
Description: Based on the inspector's review of four our of thirty-eight staff records, the center failed to ensure an out of state search of the central registry was requested when a staff member confirmed on a signed sworn statement, they had lived in another state in the past five years. Evidence: The record for staff member #1 has a signed sworn statement indicating they have lived outside of Virginia in the past five years. The staff member has a documented date of employment as 9/1/17 and signed sworn statement dated 9/5/17. There is no documentation of a search of the central registry request from the state the staff member indicated as having lived in over the past five years.

Plan of Correction: The PD will make the request by the next program day. Documentation of the request will be sent to the licensing inspector within ten days of today's inspection.
