Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Disclosure Statement for new staff hired after 7/1/2017 and requesting out of state Central Registry checks for staff who have lived outside the State of VA within the last five years. The model SDS form is online at VA DSS, a link to the Adam Walsh site to assist with out of state central registry forms as well as contacting VA State Police to research a submitted CRC. VDSS will be paying for initial child care fingerprint checks until 9/20/18. Please read and review all correspondences regarding the fingerprint roll out.

Comments:
An unannounced monitoring visit was conducted from 9:50-11:15. Thirty four children were present with direct care staff in three classrooms. Observed work time with Montessori materials, children preparing snack, transitioning to outside playtime and emergency preparation. Lunch served today was turkey sloppy joes, vegetables, fruit and rolls. Reviewed three staff records, four children records, medication storage and authorizations. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on staff statement, the center would be unable to ensure care for two children as they could not follow the allery action plan signed by the physician. Evidence: Child #1 and Child#2's action plans called for an antihistamine to be on-site in case of an allergen exposure. The center has requested the medication from the parent but it is not on-site.

Plan of Correction: Parents of both children were informed to provide antihistamine as wellas Epi-pen by the end of the week.

Standard #: 22VAC40-185-550-M
Description: Based on review of injury forms 5 of 8 forms were incomplete. Evidence: 1. One form was missing future action to prevent reoccurrence. 2. Three forms were missing an actual time the parent was notified (pick-up was listed rather than the time) 3. One form was missing a second signature

Plan of Correction: Injury form was amended to show time when parent was informed by the staff. Consulted to always include plan to prevent injuries.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on record review, one of three staff records was missing a criminal history check (CRC) within 30 days of employment. Evidence: Staff #3 was hired 5/22/2017 and a completed CRC was not returned to the facility within 30 days.

Plan of Correction: State Police was called and they will send a copy of CRC which was not received in time.

Standard #: 22VAC40-191-60-C-1
Description: Based on staff interview and record review, an original Criminal History Record report was not received within 30 days and the employee continued to work. Evidence: Staff #3's CRC has not been received as of the date of the inspection and the center did not follow up after the 30 days to ensure the submission had been received by VA State Police. The employee continued to work without the original CRC on file.
