Comments:
An unannounced monitoring inspection was conducted today from approximately 8:45am-1030am. There were 61children in care directly supervised by 8 staff. A sample of 6 children's files, 2 medications and 4 staff files were reviewed. The children were observed eating breakfast, playing on the playground, dancing, during transition and in group play. Today the children made clay with the staff, they were engaged in learning about numbers and different textiles during this project. Zero new staff has been hired since the last inspection and 2 substitutes were present. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were found to be incomplete. Evidence: 1) Child A and B were missing the "first date of attendance"

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-185-260-A
Description: Based on observation, the center failed to provide an annual fire inspection report. Evidence: The last documented fire inspection expired 9/1/2016.

Plan of Correction: The director has attempted to get a recent fire inspection from the school since the beginning of the school year. The school will schedule an appointment for a new fire inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation, electrical outlets did not have a protective cover. Evidence: 1) Two outlets in room 134; 1 electrical outlet covered by tape near the phone above the children's cubbies, and 1 between the computer desk and refrigerator did not have protective covers. 2) An outlet above the children's cubbies near the phone in room 138 did not have a protective cover. 3) An outlet between the refrigerator and the desk in room 137 did not have a protective cover.

Plan of Correction: I will purchase more protective covers and cover all unused electrical outlets.

Description: Based on medications reviewed, medication was not labeled with the child's name. Evidence: The medication of Child C located in room 138 was not labeled with the child's name.

Plan of Correction: Medication will be labeled.
