Discussed immunization records for children and background checks.

Comments:
An unannounced monitoring inspection was conducted from 11:15am -1:00pm. 41 children were in direct care of 10 caregivers. Children were observed returning from the playground, preparing for lunch, handwashing, eating lunch, and getting ready for nap time. A selection of staff and children records, injury report log and the emergency preparedness plan were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on observation, areas inside the center were not maintained in a safe and operable condition. Evidence: Two soft chairs in the three to five year old classroom had visible tears and holes, causing exposure to the stuffing of the chairs.

Description: Based on record review and staff interview, the center allowed staff to continue employment and supervise children without obtaining an out of state central registry from outside the Commonwealth of Virginia. Evidence: Staff # 1's central registry form for the state of Virginia and updated Sworn Disclosure Statement, completed on October 4, 2017, showed that the staff person had lived outside of Virginia in the last five years. Center management did not submit the required out of state central registry by September 1, 2017. Staff # 1's hire date is 8/18/14.
