22VAC40-191 Background Checks (22VAC40-191)

Today's visit was a routine monitoring inspection. The inspection began at 10:15am and concluded at 1: 45pm. There were forty-two children present with seven direct care staff member's. During the inspection morning snack time was observed, the playground and outside time was viewed, five out of twelve staff records were reviewed, five out of fifty-eight children's records were reviewed, all four classrooms were observed, nap time was observed, emergency supplies were viewed, and some policies and procedures were reviewed. A background check violation regarding violations was cited during today's inspection. Follow-up regarding the background check request is required within ten and evidence once the background check is received completed. Thank you for time. Standard #: 22VAC40-185-130-A
Description: Based on the inspector's review of five out of fifty-eight children's records, the center failed to ensure a child had documentation of receiving immunizations before their first day of care. Evidence: The record for child #4 indicated a start date of 8/2/17 and the immunization record was date 10/24/17, after the child began care.

Plan of Correction: In the future children will not be allowed to begin the program without proof of immunizations.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on the inspector's review of five out of twelve staff records, the center failed to ensure the record for a staff member had an updated search of the central registry. Evidence: Staff member #1 has a completed search of the central registry dated 1/27/12. There is documentation of a submitted CPS background check request in 2014 and was not completed due to an incomplete request and follow-up was not documented . The CPS background check for staff member #1 is expired according to the date of completion.

Plan of Correction: The PD will submit a new CPS request by the next program day.
