An unannounced monitoring inspection was completed on11/1/2017. There were 22 children were in care at the center, ages two years through five years old with four staff supervising the children. The inspector arrived at the program at 10:00 A.M. and departed the program at 12:30 P.M. Four staff's records and five children's records were reviewed. Informal interviews were held with all staff members and there were interactions with most of the children. A walk-through of the center and house was done. The children were observed in an inside activity, art time, music time, free play, outside time and lunch Discussion with staff about: completing the new sworn statement that was required with the code change 7/1/2017, completing a record for staff # 1, and diapering procedures and location of changing table versus the location of a sink.
Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, the center failed to ensure that all records were complete. Evidence: 1. The record for children # 2 and 4 did not have allergies completed on the enrollment form. 2. The record for child # 3 did not have complete address for both of the required emergency contacts. 3. The record for child # 4 had the parents as the emergency contacts. Plan of Correction: The administrative assistant contacted the parents to obtain the missing information and will monitor all records.
Standard #: 22VAC40-185-280-B Description: Based on observation of of the large gathering room, the center failed to ensure that all chemicals were locked. Evidence: 1. There were several containers of Clorox wipes unlocked on a high shelf that was inaccessible to the children. 2. The diapering chemicals were unlocked on a high shelf in the two year old room. Plan of Correction: A hook lock will be placed on the door today. All chemicals will be locked.
Standard #: 22VAC40-185-500-B Description: Based on observation of the diapering area, the center failed to ensure diapering requirements were met. Evidence: 1. Child # 5 was using cloth diapers and they were disposed on in a reusable zipped bag. The method of disposal was not in a method where the diaper or the caregivers hand did not touch the container. 2. The changing table was cleaned with wipes after use, instead of being cleaned with soap and water and sanitized. Plan of Correction: The staff will purchase a disposal system that will not allow for touching of the container.
