An unannounced monitoring inspection was conducted today. There are 6 children present with 2 staff. An additional staff person is on a field trip with another child. 3 staff (basic components only) and 4 children's records were reviewed See violation notice for missing items. Medications were reviewed and were in compliance. Children were observed washing hands, eating lunch and working on a variety of projects in the room. Standard #: 22VAC40-185-130-A Description: Based upon a record review, one child's file does not contain documentation of immunizations prior to the first day of attendance. Evidence: Child #1 (Date of enrollment: (8/24/2017) does not have any immunization record on file. Plan of Correction: I will go to the base school and get a copy of the form.
Standard #: 22VAC40-185-150-A Description: Based upon a record review, one child does not have a completed physical on file, past 30 days of enrollment. Evidence: Child #1(DOE: 8/24/17) does not have documentation of a completed physical on file. Plan of Correction: I will go to the base school and get a copy of the form.
Standard #: 22VAC40-185-540-C Description: Based upon a review of the first aid kit, the kit does not contain all required components. Evidence: There is no second triangular bandage available. Plan of Correction: I will buy one or make one.
Standard #: 22VAC40-185-550-D Description: Based upon a review of available documentation, monthly emergency drills have not been documented. Evidence: There is no drill documented for the month of October 2017. Plan of Correction: The drill was conducted, just not written down. The drills will be documented as they are conducted.
