An unannounced monitoring inspection was conducted with center director and center staff. There were 48 children present, ranging in age from 2 months to school age, with 10 staff actively supervising. The children were having afternoon snack, playing with age appropriate toys, and playing group games in the Hope Center "indoor gym". The playground was not reviewed today due to inclement weather. Nine medications for six children were reviewed - please see Supplemental Summary for missing items. The last health inspection was conducted 10/12/2017. The last fire inspection was conducted 2/15/2017. Three new staff files were reviewed and are in compliance. Standard #: 22VAC40-185-280-B Description: Based on observation, the facility failed to keep all hazardous substances such as cleaning materials in a locked place. Evidence: a bottle of sanitizer was sitting on the sink counter in the threes room. Plan of Correction: The center director locked the sanitizer during today's inspection.
Standard #: 22VAC40-185-500-A Description: Based on observation, the facility failed to wash all children's hands with soap and running water after toileting. Evidence: after changing a one year old's diaper in the infant room, the staff person put him on the floor. When the licensing inspector asked if the staff planned to wash the child's hands, the staff person replied "Am I supposed to?" Plan of Correction: Staff has been retrained on proper hand washing and observation of these practices will continue to be made.
Standard #: 22VAC40-185-510-C Description: Based on review of nine medications for six children, the facility failed to have a method to prevent use of outdated medication. Evidence: one emergency medication for Child B expired 10/31/2017. Plan of Correction: Medication expiration dates are reviewed monthly and parents are given ample time to replace. Outdated medication has been and/or will be replaced immediately.
Standard #: 22VAC40-185-520-C Description: Based on observation, the facility failed to have diaper ointments/creams out of reach of children. Evidence: in the threes room, two diaper ointments were located in the children's cubbies, which were within reach of the children. Plan of Correction: The diaper ointments/creams were placed on a high shelf in the bathroom, out the of the children's reach during today's inspection.
