Comments:
An unannounced monitoring inspection was conducted today with staff. Thirty two children, between the ages of 13 months and 6 years, were present with five staff. The Licensing Inspector was able to observe outside time, transition to pick up time, lunch which took place outside on picnic tables. There were no medications on site. Injury/accident log was reviewed today as well as five staff and children's files. Arrival time: 11:15 am Departure time: 12:50 pm

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of five children's files it was determined that one child did not have an immunization record on file at the time of enrollment. Evidence: Child A enrolled in 9/17 and there was no immunization record on file.

Plan of Correction: We will get the immunization record from the parents.

Standard #: 22VAC40-185-140-A
Description: Based on a review of five children's files, it was determined that there was no physical on file within 30 days of enrollment. Evidence: Child A enrolled in 9/17 and there was no physical on file.

Plan of Correction: We will obtain the physical from the parents

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that there insufficient resilient surfacing under and around play equipment. Evidence: There was no resilient surfacing under and around the wooden climber and the portable slide on the toddler playground.

Plan of Correction: We will place this equipment off limits until brought into compliance.
