Comments:
An unannounced monitoring inspection was completed on 11/16/2017 from 1:40pm to 3:00pm. At the end of the inspection there were eight children in care with four staff. Children were observed during rest period. The sample size was 5 children's records and 5 staff records. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and interview, it was determined that the provider did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: On the playground, there was a plastic Little Tykes picnic table that had areas of cracking and could cause an injury due to sharp edges.

Plan of Correction: The facility responded: The broken picnic table will be removed. Staff has been reminded to complete daily check of playground and remove any harmful items at that time.
