Comments:
An unannounced monitoring visit was conducted today beginning at 11:40am. 55 children were directly supervised by 8 staff members. Ratios were in compliance. The physical space, 5 children's records, 5 staff records, evacuation drills, the Emergency Preparedness Plan and medications were reviewed. Required documents were posted. The children were observed eating Thanksgiving meals and were dressed in crafts they made for Thanksgiving. Lunch today consisted of turkey, gravy, sweet potatoes, cranberry sauce, rolls and milk. The children were eating "family style" and practicing their manners. Interactions between staff and children were positive. The space was clean and well organized. Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 out of 5 children's records did not have documentation of viewing proof of the child's identity and age. Evidence: Child #1 was missing the page with the Director's signature.

Plan of Correction: We don't receive children without identification. It's possible the document with the signature was accidently returned to the parent. (In the presence on the LI, the director sent the parent an email requesting the documentation again.)
