An unannounced monitoring inspection was conducted on November 8, 2017, from approximately 10:40 AM to 12:15 PM. Five staff were present with 13 children. Children were observed freely playing in classrooms and eating lunch. Five children records were reviewed and 5 staff records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, health inspection report, emergency supplies and required positing. No medication is given at the center. The exit interview conducted with the owner/program director.
Standard #: 22VAC40-185-140-A Description: Based on 1 of 5 records reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the complaint investigation conducted on November 8, 2017, the record for child #3 did not have a physical examination. Standard #: 22VAC40-185-160-C Description: Based on record review, the licensee did not ensure staff had documentation of tuberculosis screening with in the past 2 years. Evidence: During the inspection conducted on November 2, 2017, staff #5's last TB test was completed on October 16, 2015 which is more than 2 years old. Plan of Correction: Staff recieved
Standard #: 22VAC40-185-270-A Description: Based on observation, the licensee did not ensure the area inside and outside was maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on November 08, 2017, the following was observed : -An accumulation of dust in the building . Dust was visible on the window -sills, on the baseboard and on the vent in the hallway. -In the building there were multiple areas with peeling paint. In the infant classroom, there was peeling paint on the wall by the cribs and peeling paint on the base board through out the building. - In the building there were multiple areas of rust. In the infant classroom the door to exit outside is rusting and when the inspector touches the door a red color residue came off on to the inspector's hand. In the bathroom by the three and four year old classrooms, there were there was rust on the right and left handicap grab bars when touched by the inspector a red color residue came off onto inspector's hand. - In the toddler's classroom, there was a tripping hazard the floor was raised by the diapering area and sink about a inch. The raised area is not visible which caused a tripping hazard. Plan of Correction: Will be corrected all.
