Comments:
An unannounced monitoring inspection was conducted from 12:30pm-2:45pm. There were 68 children in direct care of 18 staff members present. Children were observed napping, infants were observed getting fed, playing with toys and getting ready for nap. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment inside the center were not maintained in a safe and operable condition. Evidence: Two toddler soft chairs in the toddler classroom were ripped and had tears, exposing the inside filling of the chair.

Plan of Correction: We will order new chairs.

Standard #: 22VAC40-185-570-E
Description: Based on observation, Infant bottles with milk were not dated. Evidence: 1. 3 milk bottles for Child # 1 did not have the date on them. 2. 2 milk bottles for Child # 2 did not have the date on them.

Plan of Correction: Bottles were dated during the inspection.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and staff interview, the center did not obtain a copy of the results of a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: 1. Staff #1 lives in Maryland and has lived in Maryland within the last 5 years. An out of state central registry was not submitted for Staff # 1. Staff # 1's hire date is 2/15/13. 2. Staff # 2 has lived in Utah within the last five years. An out of state central registry was not submitted for Staff # 2. Staff # 2's hire date is 7/14/14.

Plan of Correction: I will submit the central registry forms for both employees.
