Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which activities, group time, outdoor time, music time, small and large motor activities, and lunch time were observed. Staff and children records and other documentation were reviewed. There were 29 children in care, aged 8mos-4yrs, with 5 staff, office staff, and kitchen staff. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 9:15am and concluded at 12:30pm.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to proof of immunizations. Evidence: Child record #4, enrollment date 8/7/17, did not have proof of immunizations as required.

Plan of Correction: This document will be obtained as soon as possible, this was an oversight. Files will be continue to be reviewed for compliance.

Standard #: 22VAC40-185-510-E
Description: Based on review of medications the facility failed to maintain compliance with this standard as required relating to labeling of medications. Evidence: Medication for child B, epi-pen, did not have the pharmacy label on the box as required.

Plan of Correction: The label will be obtained and placed on the box as required. Medications will continue to be reviewed for compliance.
