Today we discussed background check regulations. Information was provided in hard copy.
An unannounced monitoring inspection was conducted from 3:15pm through 5:00pm. There were 18 children ages five to ten years old in care with two staff. Children and staff records were reviewed. Complete staff records are housed at a central location and will be reviewed at a later date. Children were observed in teacher and self-directed activities including homework, crafts, snack/dinner, restroom routines, hand washing and reading. Violations regarding tuberculosis screening and child physical were cited and appear on the violation notice.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview the center failed to ensure that each child has a physical examination by or under the direction of a physician within 30 days of enrollment. Evidence: 1 - The record for child 1 does not contain a physical report. a. The documented first date of attendance is 9/13/17. 2 - Staff 1 confirmed that there is not a physical report on file for child 1 Plan of Correction: Center management will review each child's record to ensure that they all contain all required information. Any missing information will be obtained from the parents.
Standard #: 22VAC40-185-160-C Description: Based on staff record review and interview the center failed to ensure that at least very two years from the date of the first initial screening or testing staff members obtain and submit the results of a follow-up tuberculosis screening. Evidence: 1 - The record for staff 1 contains a tuberculosis screening that is dated 8/11/15. The screening is past due for update. 2 - Staff 1 stated that she did not have documentation of an updated tuberculosis screening. Plan of Correction: Center management will ensure that all staff records contain current tuberculosis screenings/tests.
