An unannounced complaint investigation was conducted with the owner and director beginning at 11:00 AM and ending at 12:50 PM. The investigation was regarding allegations of inappropriate supervision for children in care, staff-to-children ratios, lack of care, protection, and guidance, and failure maintain written documentation of injuries on the day of occurrence. One parent interview and four staff interviews were conducted. One child's record and two staff records were reviewed and in compliance. The sign in/out sheets for children's attendance in the three year old classroom for one month were reviewed and in compliance. It was determined the allegation regarding inappropriate supervision, staff-to-children ratio, and failure to maintain written documentation of injuries was not valid. It has been determined the allegation regarding lack of care, protection, and guidance was valid. Description: Based on a review of documentation and staff interview, the center staff did not always ensure the care and protection of the children being supervised. Evidence: On October 18, 2017, Child A sustained an injury which left a red mark on her chest area from being given warmed cranberry sauce without it being properly cooled before served to the children in the three year old classroom.

Plan of Correction: The teacher tasted the sauce before serving and it was an acceptable temperature to her - warm. The employee is no longer working for the center. A request for appropriate food temperature was requested but a definitive answer was not able to be obtained. After discussion, it was agreed upon that checking temperature of food to the inside of the adult wrist would be acceptable. If the food was warm, then it can be served.
