22 VAC 40-185-550-M: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

Comments:
An unannounced monitoring inspection was conducted with the director beginning at 12:50 PM and ending at 3:50 PM. There were thirty-six school aged children in care of three staff during the inspection. The infants were observed playing with manipulatives and being held by staff members. Three infants were observed sleeping in cribs. Diapering and hand washing procedures were observed. The older children were served lunch during the inspection which consisted of grilled cheese sandwiches, vegetable soup, and milk. The children were also observed asleep or resting quietly on their cots during nap time. Six children?s records and five staff records were reviewed and in compliance. There were no medications on site during the inspection. The outside recreational areas were not inspected due to inclement weather. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent lock-down drill was documented as January 26, 2017. The most recent emergency escape drill was documented as October 20, 2017. The most recent fire inspection was documented as October 2, 2017. Standard #: 22VAC40-185-270-A
Description: Based on observation, not all of the inside areas of the center were maintained in a clean condition. Evidence: The boys bathroom in the school-age classroom had a strong smell of urine. There was also visible urine stains on the side of the toilet.

Plan of Correction: The classroom was vacant during time of inspection. Bathroom is cleaned daily.

Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous substance were kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a stray bottle of disinfectant left on a table in the school-age classroom.

Plan of Correction: The classroom was vacant during time of inspection. Sanitizer was placed in locked cabinet before licensing inspector left the building.
