A Subsidy Health and Safety monitoring inspection was conducted on 11/08/2017 to review supplemental health and safety requirements. This inspection began at 11:40am and ended at 3:15pm. There were 11 staff with 43 children in 2 classrooms at the time of the visit. At the start of the inspection, children were observed playing outside, washing hands, eating lunch, pottying and napping. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Standard #: VENDSUB-000-006-B-11
Description: Based on record review, the center did not obtain a written allergy care plan for each child with a diagnosed food allergy. Evidence: Child #2 is diagnosed with a food allergy and there is no allergy care plan from the child's physician instructing staff of steps to be taken in the event of a suspected allergic reaction.

Plan of Correction: We will obtain a written allergy care plan for child.

Standard #: VENDSUB-000-012-B
Description: Based on record review and interview, all staff members have not completed the Virginia Preservice Training. Evidence: Staff #8 and Staff #9 did not have documentation of completed Virginia Preservice Training The center Director confirmed that these two staff members have not completed the training.

Plan of Correction: Staff will complete Virginia Preservice Training.

Standard #: VENDSUB-000-012-F
Description: Based on record review and interview, all staff members do not have current CPR and first aid training. Evidence: Staff #2 and Staff #6 did not have documentation of current CPR and first aid on file. The center Director confirmed that there was no documentation available for these two staff.

Plan of Correction: Staff will obtain CPR/First Aid certification and/or we will provide documentation of said certifications.
