Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:00am - 1:30pm. There were 17 children in care: one infant and three toddlers with one staff, seven 2 year olds with two staff and six preschoolers with two staff. One additional staff was also present. The children were observed at free play, working puzzles and during naps. Five staff's records and five children's records were reviewed. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record reviews, the center failed to obtain immunization documentation before child's first date of care. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #1) did not contain documentation of immunizations.

Plan of Correction: The director will ask the child's parent to provide a copy of the child's immunization for the child's record.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on record reviews, the center failed to obtain a repeat background check. Evidence: 1. The inspector reviewed five staff's records. One of five records reviewed (staff #5) contained a criminal history record check that was more than three years old. The check was dated 11/03/2014.

Plan of Correction: The director will complete and submit a request for a new criminal history record request today for staff #5. A copy of the request will be emailed to the licensing office.
