63.2 Facilities and Programs..

Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:20 a.m. until 11:45 a.m. There were 17 children in care with 3 staff supervising at all times. The children were observed washing hands, going to and eating lunch, and playing outside. Five children?s records were reviewed. No medications are being administered at this time. Areas of non-compliance are identified on the violation notice. If you have any questions, please contact your licensing inspector at 276-698-9981. Thank you for your cooperation.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain all the required information in the children's records. Evidence: 1. The inspector reviewed five children's records. Three out of five records had missing information about parent employment. Record #1 did not have listed the parent work place. record #2 had no work phone # listed. Record #3 had no work place listed for the parent. 2. The staff also tried to locate the missing information and could not find it.

Plan of Correction: Staff will get the missing information from the parents.

Standard #: 22VAC40-185-270-A
Description: Based on observation and measurement, the center did not maintain an area on the playground in a safe condition. Evidence: 1. The inspector observed an area in a landscaping timber measuring 6 feet long that was splintered and broken off and it had two metal stakes exposed approximately one inch sticking out vertically.

Plan of Correction: Staff is aware of the issue and will make the repairs.
