Comments:
An unannounced monitoring inspection was conducted on Monday, December 4, 2017 to determine the program's compliance with licensing standards. The licensing inspector was on site from approximately 3:00PM until 5:35PM. During the inspection there were a total of 104 school age children being supervised by 10 staff members. Children were observed taking bathroom breaks and washing their hands, eating, playing outside and completing homework. The afternoon meal consisted of spaghetti, broccoli, apples and milk. All areas used by children were inspected to include the main hallways, children's bathrooms, gym, cafeteria and playgrounds.

The following information was reviewed during the inspection: daily attendance, emergency drill logs, emergency supplies, menu, medication and medication authorization forms, parent agreements, and staff training information. During the inspection, 10 children's records and 12 staff records were reviewed. The information required for staff members was present at the facility. Additional components of staff records are stored off site and will be reviewed by licensing inspectors at a later date. Standard #: 22VAC40-185-140-A
Description: Based on review of 10 children's records, one child did not have a physical exam within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Child #10 (enrollment date 09/27/17) did not contain a physical exam. Each child is required to have documentation of a physical exam prior to or within 30 days of attending the facility.

Plan of Correction: We will continue to review children's records and reach out to the parents to request the physical exam.

Standard #: 22VAC40-185-60-A
Description: Based on review of ten children's records, two records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the records for Child #1 and Child #7 did not contain the physical address for the two emergency contacts listed in the children's records.

Plan of Correction: Corrected during the inspection.
