22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 10:45am-2:00pm. There were 55 children (infants-4yrs.) directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, painting, singing "Frosty the Snowman" and "The Wheels on the Bus", playing outside, and eating lunch. Today's lunch consisted of whole grain macaroni and cheese, peas and carrots, apricots and milk. Hand-washing procedures were also observed. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. There was an abundant supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, 1 of 5 children files is missing documentation that a proof of of identification and age were verified within 7 days of the child's first day of attendance. Evidence: Child#4's first day of attendance was 9/4/17, the child's file did not contain documentation that their proof of identity was verified by a staff member. Plan of Correction: I will request the information from the parent.
Standard #: 22VAC40-185-240-A Description: Based on review of staff records, the center did not have documentation that a staff member received training by the end of their first day of assuming job responsibilities. Evidence: Staff#3: did not have documentation that the individual had received required staff orientation by the end of the first day of assuming job responsibilities. Staff member was hired in 10/2016. Plan of Correction: We will find the form and we will make sure that is placed back in her file.
Standard #: 22VAC40-185-270-A Description: Based on observation, not all areas inside the center were maintained in a clean and operable condition. Evidence: In the Preschool classroom, there is a strong smell of urine coming from the drain in the boys bathroom. Plan of Correction: I will place a work order today to fix the problem.
Standard #: 22VAC40-185-290-3 Description: Based on observation, one unused outlet was missing a child-resistant protective cover. Evidence: In the preschool room, by the children's cubbies an outlet was missing a child-resistant protective cover. Plan of Correction: Corrected during the inspection.
